Among others: Patient Name, Medication Dosage, Physician, Charge, Charge Code and Control Number. You can customize this form to meet your needs. Just let us know what you need to Change. Duplicate Labels with a log that keeps a record of all Medication that were dispensed. There are 14 Medication Labels per sheet. Each label is 1 1/2" x 3 9/16". There are 1400 Medication Labels per package plus a log sheet.
